Box 2290, Cordova, Alaska 99574 xxx-xxx-xxxx Project Director: Kari Collins xxxxxxxxxxxx@xxxxxxxx.xxx www.nveyak.org Service Area ID#: 002 Alaska Congressional District HRSA §330 CHC Funding: $1,840,661 History & Community: The Native Village of Eyak (NVE) is a federally recognized IRA tribal government that is a direct co-signer of the Alaska Tribal Health Compact. NVE has operated the Ilanka Community Health Center since 2000 and became a §330 HRSA Community Health Center in 2005. Ilanka CHC offers comprehensive primary health care services to all residents and visitors in Cordova, Alaska. Cordova is a small, remote fishing community of 2,545 year-round residents on the eastern shores of Prince William Sound. The population doubles during the summer with an influx of commercial fisherman, cannery workers, sports fishermen, adventure tourists and seasonal workers. Ilanka CHC’s service area includes the entire populated area of this census tract. Major Health Care Needs and Barriers: Cordova is geographically isolated, with access only by air or ferry. The area is classified as sparsely populated. Flights to Anchorage, along with related travel expenses of lodging and local transportation are expensive. Due to the remote location, it is imperative that high quality, affordable medical care be readily available locally. Community health needs are assessed on an on-going basis. Major areas of need identified include mental health and substance abuse services, affordable and accessible health care, wellness and nutrition services, and chronic care management. Services to Address Needs: Ilanka CHC offers comprehensive medical and behavioral health services and ancillary services, including Nutrition services, care coordination and case management. Ilanka CHC also offers enabling services. This includes Assistance to patients with local transportation and coordination of travel to Anchorage; Information on and assistance with applying for ACA insurance plans and subsidies; Assistance with Medicare and Medicaid enrollment; and other non-clinical support. All services are provided at a single fixed-site location conveniently located in downtown Cordova. Proposed Project: The 2021 UDS Report recorded services provided to 1,231 patients with 4,143 medical visits, 1,054 mental health visits, 192 SUD visits, and 52 optometry visits. This application projects calendar year 2024 activity at a total of 5,915 visits for 1,377 patients.
