Tolentino E-Mail: xxxxxxxxxx@xxxxxx.xxx Contact Phone Numbers: xxx-xxx-xxxx Ext. 4443/4765) Contact Fax Number: xxx-xxx-xxxx General Overview of HIV Epidemiology in the Entire Designated Service Area Selected: Healthcare Integrated Program Services, Inc./Programa CIS is a non-profit organization established within the Ryder Memorial Hospital facilities in Humacao, Puerto Rico. The program and the experienced staff have been providing services to the community for the past 33 years (30 years with Ryder Memorial Hospital as program’s recipient and three years as Healthcare Integrated Program Services, Inc. (HIPS)\Programa CIS as recipient). HIPS provides Early Intervention Services (EIS) to HIV/AIDS participants within Puerto Rico’s southeastern region with Ryan White funds collaboration. HIPS is offering services to 14 rural counties: Aguas Buenas, Caguas, Cayey, Ceiba, Cidra, Fajardo, Gurabo, Humacao, Juncos, Las Piedras, Maunabo, Naguabo, San Lorenzo and Yabucoa. HIV/AIDS participants receiving our services are low-income, vulnerable, medically underserved people living with HIV/AIDS; including women, pregnant women, infants, children, youth, MSM, homeless person and others. Target Population(s) to be Served: One of our program objectives is to offer high quality of services to at least 370 VIH/AIDS program participants. From the period started on April 1, 2023, through March 31, 2024; a total of 393 individuals received services in our program. HIPS/ Programa CIS participants were 99% Puerto Rican, 67% males and 33% females. Most people served were from 45 – 64 years old (62%), followed by people >65 years old (25%). A total of 99% had permanent residences, with a major risk factor of heterosexual contact population (67%), followed by MSM (27%) and IVDU (10%). Description key services to be supported by this request, amount requested and target populations: Healthcare Integrated Program Services, Inc. (HIPS)/Programa CIS include the following services: HIV counseling and testing, referral and partner counseling services, medical evaluation and clinical care, oral health, treatment adherence, outpatient mental health, specialty care, medical nutrition therapy, medical case management, referral clinical research, health and risk education and support services. Our overall program goal is to increase accessibility and availability of high-quality early intervention services to participants at-risk or with HIV/AIDS. HIPS/Programa CIS major objectives include primary care services and treatment to an average of 25 new HIV/AIDS clients and 370 follow up clients. Also, to offer HIV counseling and testing services to an average of 850 individuals. Offers treatment adherence services to an average of 370 HIV/AIDS clients. Provides nutritional and mental health to 300 HIV/AIDS clients. Provides oral health care services to a maximum of 250 HIV/AIDS clients. Through these services we expect to achieve the following results: 95% service retention, 95% viral load suppression, 95% ARV treatment compliance and 370 patients in compliance with their medical appointments. The Healthcare Integrated Program Services, Inc. is interested in receiving $548,064 funding from Ryan White Part C HIV Early Intervention Service, including preference funds, to keep providing quality service to our community and expands EIS for minority and underserved population within Puerto Rico Southeast region highly impacted by HIV/AIDS.
