(FCH) is a 501(c) 3, non-profit organization and the Sponsoring Institution for Family Medicine education since ACGME (Accreditation Council for Graduate Medical Education) accreditation in 1974. In 1975, FCH’s Family Medicine Residency in Boise, Idaho starting training their first class of Residents and 2009, started the Family Medicine Residency in the Magic Valley (Twin Falls/Jerome, Idaho). In these 47 years, the program has grown tremendously to meet the challenges of our communities and state. The FCH Sponsoring Institution has evolved to two additional ACGME accredited Family Medicine Residency programs, one ACGME accredited Pediatrics Residency program, and two ACGME accredited Fellowship programs in Sports Medicine and Geriatrics. FCH has three additional non-ACGME accredited Fellowships in HIV & Viral Hepatitis, Obstetrics, and Rural Emergency Medicine. FCH’s main primary care training location is located in Boise, Idaho and the Residents utilize our decentralized health system comprised of nine FQHC clinics located in Ada and Canyon counties. FCH offers excellent clinical training with superb patient volumes in all clinical areas and provides training to perform all office-based and inpatient procedures needed to feel comfortable in a broad-spectrum practice. Behavioral Health training is at the core of our Residency training program. We have integrated behavioral health faculty and LCSWs into our clinic system. This allows for warm handoffs around behavioral issues (mental health, substance abuse, motivational interviewing, etc.). Success in any program is a team effort and we are fortunate to have the outstanding support of dedicated faculty and staff as well as tremendous hospitals in the Treasure Valley and Magic Valley that support these programs. Additionally, FCH utilizes 42 different communities, including twenty-five hospitals, and hundreds of physicians, and their staff throughout the state to help form the network of rural training sites that ensure outstanding broad-based rural family medicine training opportunities. FCH served 30,113 Patients in calendar year 2022; 10% of whom were uninsured and 39% of whom were recipients of Medicaid, at their time of service. Over one-third of FCH’s Patients, 36%, live at or below 200% of Federal Poverty Guidelines (FPG). Approximately one in three of FCH’s Patient population are living in poverty which is at or below 100% FPG. FCH serves a socioeconomically disadvantaged Patient population. Therefore, enabling services such as insurance enrollment and health literacy education are essential services offered to Patients at all service sites. Additionally, FCH is an MHC (Migrant Health Clinic) grantee, having served 316 Migrant and Seasonal Agricultural Workers (MSAW) in 2022. FCH is also a medical point of entry for refugees resettled in Boise, Idaho and provides comprehensive primary and mental health services to refugee patients and help refugees assimilate and recuperate following their traumatic loss. 6.
