We are filing this SAC Application to continue funding as a Health Center Program award recipient in the Birmingham, Alabama metropolitan statistical area to serve the medically underserved. We have 3 sites in the Woodlawn and Chalkville neighborhoods. We operate a primary care/family health medical clinic in both of these neighborhoods with a general dentistry clinic in the Woodlawn clinic location. We also operate a behavioral health clinic and pharmacy in Woodlawn next to the Woodlawn Clinic. In early 2022, we acquired a building adjoining our Woodlawn Clinic property to be renovated for administrative and clinical space. In addition, we are renovating a mobile unit for medical and preventive dental services in order to serve patients in our Service Area who have difficulty accessing either of our locations. We began providing medical telehealth services in 2020 and continue those services today. The State of Alabama is the 6th poorest State in the nation per the 2022 Poverty Data Sheet provided by Alabama Possible, a nonprofit organization breaking down barriers to prosperity. On July 1, 2021, the poverty rate in Birmingham was 24.7% per the ACS Quick Facts from the U.S. Census Bureau. The majority of our patients are uninsured or Medicaid-insured patients with most of these falling into the 100% or less category of HRSA's Federal Poverty Guidelines. In 2021, we served 17,504 unique patients and are on track to serve that many or more in 2022 which will more than meet our patient target for HRSA. We are renovating our clinical areas to provide more exam rooms and adding additional medical providers and behavioral health providers to meet the needs of our patients. The need in our Service Area is great and we wish to continue the work we have begun.
