Compass Health Network (Compass) is a nonprofit health care organization that provides a full continuum of behavioral health services as well as primary and dental health services throughout Missouri. As a Department of Mental Health Prevention provider, Compass’ mission is to engage and empower communities to promote and support community wellness. Compass’ continuum of care extends to collaborative partnerships throughout the communities it serves. Community. The service area includes 46 counties in Missouri and 334 zip codes. The HRSA-defined zip code service area population has a total of 3,732,258, of whom 12.24% were in poverty and 28.99% were low income per with approximately 28.99% being uninsured UDS Mapper. In 2021, Compass served a total of 82,176 patients including 33,615 medical patients and 31,091 dental patients. Target Population. The service area includes 1,050,990 low-income individuals (UDS Mapper); and further includes multiple Health Professional Shortage Areas for primary, dental, and mental healthcare. Compass Health targets the low-income population, and fills a significant void for behavioral health, substance use disorder, pediatric and family medicine, and dental care. Health care needs and barriers to care. Priority health care needs of the target population include diabetes, cerebrovascular disease, lack of preventive screenings, and behavioral health issues. Compass Health provides access to on-going preventive care for low income and uninsured individuals, as well as others seeking access to high quality, comprehensive health care services throughout its entire service area. Addresses need for comprehensive services. Compass Health provides a full scope of services, including primary, dental health, and behavioral health care. The proposed project reflects a staffing and leadership structure to assure timely, affordable care for the target population. Current and proposed patients, visits, providers, service delivery sites and locations, and services to be provided. As of December 31, 2021, Compass had provided services to 82,176 patients. The new patient target is 90,226 unduplicated patients, through a projected 433,434 encounters. There are currently 64 community-based service delivery sites. Providers include physicians, nurse practitioners, dentists and hygienists, and behavioral health providers along with necessary medical, enabling and facility support staff. Compass provides the full scope of care using a patient-centered medical home model.
