Applicant Title: Westside Family Healthcare, Inc.

Abstract Text: Westside Family Healthcare, Inc. (Westside) is a Federally Qualified Health Center in Delaware. Through the FY25 Service Area Competition Funding Opportunity, WFH is requesting $7,805,401 each year during the grant performance period May 1, 2025 to April 30, 2028. Westside will serve a target patient population of 24,524 unique patients annually. Westside Family Healthcare’s vision is to achieve health equity for all. Caregivers at Westside work to achieve this vision by driving forward the mission to improve the health of our community by providing equal access to quality healthcare, regardless of ability to pay. Westside Family Healthcare currently operates five clinical sites, a mobile health unit, and three administrative offices located within the cities of Wilmington, Newark, Bear/New Castle, and Dover. Westside first opened its doors in 1988 and has grown to become accredited by The Joint Commission as well as certified as a Primary Care Medical Home under The Joint Commission. Westside has maintained its status as a Federally Qualified Health Center since 1994. Westside clinicians and caregivers provide family medical, oral health, behavioral health, and enabling services to families living in underserved communities. Westside is governed by a 15-member board of volunteers and employs 240 team members to support the delivery of care.
