(FCPC), established in 2007, is a private not-for-profit Community Health Center (CHC) and a Health Care for the Homeless (HCH) grantee located in Norcross, Georgia (Gwinnett County). FCPC, through four service delivery sites, serves the residents of Gwinnett County (in Norcross and Lawrenceville (2)) and Jackson County (in Hoschton). Gwinnett and Jackson County are included in the Atlanta-Athens-Clarke County-Sandy Springs, Georgia Combined Statistical Area. The service area is, as such, primarily urban. While most of Jackson County is transitioning from a rural environment to a suburban environment, the county continues to include large areas that are considered rural. FCPC provides the full complement of comprehensive primary health care services across the lifecycles. Services, on site and through referral, include oral health, mental health, substance use disorder services, enabling services, specialty care/services, education, pharmacy assistance, and comprehensive outreach. FCPC offers services to all residents of the service area regardless of language, gender, socio-economic status, sexual orientation, gender identification, physical and mental capacity, age, religion, housing status, and the ability to pay. Particular attention, however, is directed at reaching the low-income, uninsured, underinsured, Medicaid/Medicare, and vulnerable populations. The mission of FCPC is to serve the community by seeking to meet the primary and preventive health care needs of the medically underserved by providing culturally sensitive quality, affordable, accessible healthcare and supportive services, regardless of ability to pay, race, religion, or other barriers. The FCPC service area is significantly underserved and has a host of HPSA and MUA/MUP designations. The FCPC service area, in addition to the dearth of primary health care resources, has a large low-income population, a disproportionate Black population, low educational attainment, a large uninsured population, a growing Hispanic/Latino population, and a substantial elderly population. FCPC, for almost 17 years, has been a steady complement to the health care delivery system in Gwinnett and Jackson County and has helped create a health care safety net for the low-income and vulnerable residents of the area. FCPC, through its vast and diverse experience, has gleaned a tacit understanding of the unique and complex health care needs and challenges of the target population. All programs and services have been developed in response to identified needs in the community and with a specific consideration toward health beliefs and ethnic/cultural customs. FCPC, according to the 2023 UDS Report, provided 8,931 patients with care and services. Almost 7% of patients are White, 25% are Black, and 73% are Hispanic/Latino. More than 64% of FCPC patients are best served in a language other than English, 1% of patients identify as Lesbian/Gay, and almost 87% of patients are at or below 100% of the Federal Poverty Guideline. FCPC, additionally, provided care and services to 571 patients experiencing Homelessness FCPC intends to provide 11,005 unduplicated patients with care and services in Calendar Year 2026. FCPC has, as of 09/30/2024, provided 9,005 patients with care and services.
