MDMOM is a collaboration between the Johns Hopkins University, Maryland Department of Health (MDH), and Maryland Patient Safety Center (MPSC). The overarching goal of the project is to improve maternal health quality and outcomes in Maryland. Specific objectives are to: 1) Coordinate the state Maternal Health Improvement Task Force (MHITF) and revise the Strategic Plan in line with the most recent Title V Needs Assessment The MDH-coordinated MHITF drives collaboration from ~50 traditional and non-traditional partners to eliminate preventable severe maternal morbidity (SMM) and maternal mortality (MM) in the state. It is comprised of multidisciplinary stakeholders, which meet at least quarterly to develop and assess the implementation of the 2021 Strategic Plan, and to make recommendations for maternal health activities and their prioritization in the state of Maryland. 2) Improve state-level maternal health data and surveillance We will continue to coordinate hospital-based SMM Surveillance and Review working currently with 20 of 32 birthing hospitals in the state (>70% annual births) and aiming to include additional birthing hospitals by 2028. To complement the data, we will add an equity module and conduct interviews with patients who experienced SMM; to validate the data, will re-review 10% of the cases with an outside, state-level committee in line with already approved IRB protocol. We will continue to share hospital-specific SMM data and recommendations with individual hospitals through annual reports, and to develop and widely disseminate de-identified, aggregate data from state-level MM and SMM surveillance projects through annual data briefs. 3) Promote and execute innovations in maternal health service delivery We will conduct innovative work in 3 different areas. We will continue the Telehealth Initiative for severe hypertension, through which we distribute free blood pressure (BP) cuffs and monitors for self-monitoring of BP at home; and facilitate referral for 24/7 nursing surveillance for patients at risk of or diagnosed with preeclampsia. We will continue our Hospital Equity Initiative and implement hospital-selected change ideas from our Equity Toolkit with support from expert consultants and community-based organizations that will serve as members on hospitals’ equity committees. New to MDMOM, we will develop a set of trauma-informed care activities to support patients, families, and hospital staff prevent and respond to severe maternal events. Specifically, in coordination with MDH and MHITF, we will develop, update annually, and host on the MDMOM website resource maps, and will develop, test, and disseminate a pregnancy passport for pregnant/postpartum patients to encourage patient-provider communication, referrals for needed services, and early care-seeking; with MPSC, we will facilitate the development of individualized, workable protocols for staff support after a severe maternal event; and with Northwestern University, we will implement the U.S. Preventive Services Task Force-recommended Mothers and Babies program. Mixed-methods implementation and impact evaluations will be conducted for all program components, and key results will be disseminated widely through reports, peer-reviewed articles, and meeting presentations. The project will be conducted in close coordination with HRSA, meeting all required reporting and accountability processes.
