Abstract Text: Kansas F2F HIC Center Families Together, Inc. Project Director and Contact Person 3033 W 2nd St. N www.familiestogetherinc.org Tami Allen Wichita, KS 67203-5364 xxxx@xxxxxxxxxxxxxxxxxxx.xxx xxx-xxx-xxxx ext.1007 Families Together Inc. seeks to continue as Kansas F2F with $96,750 in funding from the US Department of Health and Human Services MCHB. Kansas F2F Center ABSTRACT Problem: The Family-to-Family Health Information Center (F2F HIC) will help the families of children and youth with special health care needs (CYSHCN) throughout Kansas. We will address problems identified in the Kansas MCH 2020 report to improve health care services delivery. All services and support for CYSHCN are implemented and delivered in a culturally competent, linguistically appropriate, and accessible manner to best serve CYSHCN and their families. Goals/Objectives: Families Together, Inc. the current F2F HIC for Kansas is a parent-led and parent-run organization. F2F program goals, objectives, and activities build upon our previous experience/success include: (1) increasing the training available to Kansas families of CYSHCN to support families in shared decision making, (2) provide educational resources to families of CYSHCN to support them in navigating the complex health care system, (3) promoting Parent-to-Parent (PTP) matches among families of CYSHCN to talk to someone who has “been there”, (4) building upon meaningful partnerships with county health departments, community health clinics, medical schools, medical societies, and related service education programs (5) Promote F2F services in a culturally responsive way, (6) disseminate information about insurance options. Methodology: 1) Families of CYSHCN will receive training through conferences and workshops, 2) educational resources will be provided through our quarterly newsletter, Monday Memo, webpage and “CarING” (Care Individual Needs Guide) notebook, 3) families will be matched with other parents through our PTP program, 4) maintain current partnerships and develop and nurture new partnerships to share information, training and outreach to target populations, 5) partner with cultural brokers to promote services to underserved, diverse populations, and(6) information will be shared through web resources and CareING notebook. Coordination: The Kansas F2F Center’s continued partnerships with more than 30 agencies that serve families locally and statewide and add at least 3 more partnering agencies in each year of the award. Partnerships emphasize the use of cultural brokers to deliver culturally responsive services to underserved and at-risk populations, and community health centers and county health departments. Data sharing and collection will be conducted through our partnership with the KS Dept. of Health and Environment (Title V agency) CYSHCN program. We will work with Independent Living Centers to address health issues for transition-age youth and to serve parents who have disabilities and are caring for CYSHCN in all areas of the state. Evaluation: The KS F2F Center will assess progress monthly. Family Health Resource Specialists (FHRS) will report (1) contacts with families of CYSHCN using electronic intake system, (2) contacts and presentations to community partners, and (3) recruitment and matches for the PTP network. All participants of training and/or conferences will be asked to complete an evaluation. A random sample of families who have received services through the F2F HIC Center will be surveyed. All information will be used to evaluate the effectiveness of current activities, and design activities for later years of the project.
