Abstract Text: The Terros Health McDowell Health Center is a Federally Qualified Health Center located at 4909 E. McDowell Rd.,Phoenix, Arizona,85008. Terros Health has several Federally Qualified Health Centers in Arizona, and we adapt to meet the needs of our community and patients. We are proposing to renovate approximately 10,000 square feet to expand services for this location. The expansion will include increasing capacity of primary care providers, specialty services for adolescents, and treatment for opioid use disorders. Terros Health is committed to serving and improving quality of life and healthcare for our patients. Since 1969, we have been a grassroots substance use and recovery program, offering an open door to individuals needing care regardless of their ability to pay for services. Our mission and reach in our communities has grown over the last 50 years – developing into a fully-integrated care model that focuses on the whole person, providing integrated care and addressing health disparities. Within the zip code of the Terros Health McDowell Health Center, there are over 62,000 residents. Twenty-four percent of these residents are considered to be living in poverty, with twenty-three percent uninsured. As a Federally Qualified Health Center, we provide care for everyone, regardless of their ability to pay. Renovating our McDowell Health Center will enhance our current services to provide the highest quality of care available for individuals and families – expanding this facility to offer a fully integrated, whole person model of care. The components of this renovation will include: 1)Increasing Access to Primary Care Services; 2)Integrating Primary and Behavioral Health Care;3)Creating an Inclusive Health Center; 4)Enhancing Access to Telemedicine;5)Creating a Medication Assisted Treatment(MAT)Suite and 6)Developing Café(Children, Adolescents and Families Empowered) McDowell, our inspired model of care to support youth exposed to or engaged in substance use and with co-occurring mental health and substance use conditions.
