DOH-Orange is requesting funding in response to the Healthy Start Initiative: Eliminating Disparities in Perinatal Health to continue implementation of the Bellies, Babies, and Beyond project. Funding will support activities in urban centers in Orange County Florida. Target zip codes for project implementation are: 32805, 32808, 32811, and 32839. These zip codes have the highest concentration of Black non-Hispanic residents, poorest birth outcomes, and have demonstrated the greatest need. Target populations in both Orange County as a whole and in the specific target zip codes, exceed eligibility criteria for funding. The Bellies, Babies, and Beyond project will serve at least 700 participants annually as follows: 1) 450 participants receiving Case Management/Care Coordination consisting of 250 pregnant women, 175 participants who are pregnant, preconception, &/or interconception women, as well as infants to 18 months; and 25 fathers/partners. And 2) 250 participants who are Non-Case Management/Care Coordination participants receiving Group Based Health and Parenting Education consisting of 250 women and fathers/male partners.
