Abstract Text: The Terros Health Stapley Health Center is a Federally Qualified Health Center located at 1111 S. Stapley Ave. in Mesa, Arizona,85204. Our Terros Health Café Stapley A/R and Support project is to renovate 5,500 square feet of existing, owned, space to build out Café Stapley. Children, Adolescents, and Families Empowered (CAFÉ) is an innovative program focused on caring for high-risk and at-risk youth in our communities. Our population of focus are youth ages 12 to 17 who are at risk, or using substances, and their families, through our fully integrated care model. The Cafe concept addresses physical and behavioral health, and social determinants of health. We will create a model apartment inside of our location, including a kitchen, laundry, dining, and living room to support teaching day-to-day skills and relationships. The Café also has a learning center dedicated to the youth for education support and pre-employment development. We provide whole person care including primary medical care; living skills training; intensive substance use treatment, case management; individual, group, and family therapy; psychiatric services; and medication assisted treatment. Our target is to provide intensive services to 135 at-risk and high-risk youth annually. These community-based services allow youth to remain in Arizona and avoid placement in high cost, out of state, residential services, as well as reduce involvement in the justice system.
