LLUHEC provides high quality, community-based graduate medical education in the disadvantaged community of the Inland Empire region of Southern California to meet the region’s growing need for primary care services. Since 2012, LLUHEC has provided innovative approaches to the teaching of community-based care, and increasing the diversity of the pipeline of health professionals. LLUHEC has developed a community-based residency program that is interprofessional in training and collaborative in practice, achieving resident competency in the principals and practice of Patient Centered Medical Home (PCMH) for underserved patients and populations. LLUHEC assures that the residency training program is an integral part of patient care delivery without compromising quality of care. LLUHEC continues to be committed to training residents who are committed to serving in underserved settings experiencing health disparities. The objectives of LLUHEC are: • Residents will receive formal orientation to the community-based health setting of SAC Health; • Residents will learn specific strategies of working in the community-based health setting; • Residents will learn about health quality measures; and, • Resident/Graduate Tracking Outcomes. These objectives will be measured using formative, process and outcome evaluations. The SAC Health THC Program for Pediatrics will provide a total of twelve THC residents (four each for PGY1, PGY2, PGY3), with training in community-based health setting serving underserved patients.
