Abstract Text: ABSTRACT Project Title: Project Family STAR Applicant Name: Family Network on Disabilities Address: 26750 U.S. Highway 19 N, Suite 410, Clearwater, FL 33761 Project Director: Lisa Math Contact phone numbers: xxx-xxx-xxxx (V) xxx-xxx-xxxx (F) Email address: xxxxx@xxxxx.xxx Website address: www.fndusa.org Amount requested: $96,450.00 Need: Project Family STAR (Support, Training, Assistance, and Resources) will continue to reach out to Florida’s families of children and youth with special health care needs (CYSHCN) and professionals who serve them to provide culturally and linguistically competent health care information and support and to facilitate partnerships in shared decision-making at all levels. Goals and Objectives: The projects goals are: FND’s F2F HIC will increase the ability of families of CYSHCN, and youth with SHCN, to access community-based systems of care; increase their health care literacy; and increase their knowledge of available healthcare models and options to make good treatment decisions, receive cost effective services, and improved health outcomes. We will increase the ability of families of CYSHCN, to participate in shared decision-making and policy development, at all levels, particularly in the areas of disparity, health equity, emerging health trends, successful systems of care, the importance of having a patient family centered care model, and mental health services. FND’s F2F HIC will increase the ability of families of CYSHCN, and youth with SHCN, and the professionals who serve them, to best prepare for, and assist in successfully navigating and recovering from times of crisis or emergency. Methodology: We will reach diverse families of CYSHCN and professionals through providing them culturally and linguistically competent and appropriate training, information, assistance, and support in a variety of ways, including in-person contact and trainings (when safe), the use of online technology, webinars, websites, listservs, and social media sites. We will develop a Diversity and Inclusion Plan to help meet the needs of Florida’s very diverse population, including those who are traditionally underserved/unserved. Coordination: We will coordinate our efforts to reach families with our partners throughout the state, including Florida Department of Health, Division of Children’s Medical Services, the Florida Department of Education, the Health Planning Council of Southwest Florida (including Early Steps), Johns Hopkins All Children’s Hospital, Alpert Jewish Family Services, the University of Miami-Nova Southeastern University (NSU) and FAU Centers for Autism & Related Disabilities (CARD), the NSU Doctor of Occupational Therapy Program, the Florida PTA, the Children’s Home Network, the Family Care Council, the South Florida Wellness Network, Joe DiMaggio Children’s Hospital, and Remarkable Disability Services. Evaluation: We will use tested methods and tools that will (1) identify data sources, collect, and analyze data that will measure project outcomes; and (2) monitor and evaluate the efficiency of the project activities. FND will collect, analyze, and report data to identify measurable outcomes and strategies that are effective in serving CYSHCN, determine participant satisfaction, and track intake, services used, utilization, and costs.
